• Your medical insurance card

• Payment method: cash, check, or CC

• ALL x-rays, scans and tests relevant to your visit

• List of medications and medical history form

• A referral letter (if your insurance requires one for claims)

Patients wishing to claim from an insurance company may require a referral letter. If yours requires you to do so, please consult your General Practitioner before arranging an appointment with Dr. Roberts and bring your referral letter to your consultation appointment.
